- Your pet’s species, breed, approximate age, and current weight
- Every medication and supplement they take right now, with doses
- What you’re working on, and roughly how long it’s been going on
- Your vet’s contact details, and any recent records or a referral
- A recent photo of your pet
The form takes about 3 to 5 minutes. We confirm everything with you before any payment is requested, and there is no obligation until you approve the invoice.
